Summary:
MyCare's Community Health Workers are a mobile workforce, charged with identifying, locating, supporting, and educating MyCare patients who are unreachable, non-compliant, vulnerable, or are unaware that they need immediate, timely, and coordinated healthcare interventions.
This objective is carried out through the Touch Team and community outreach programs focused on removing healthcare barriers and managing social determinants of health (SDOH).
Effectiveness of this program lies in the ability to contact, influence, and motivate hard-to-reach individuals and to patiently coach and support them as they work on their healthcare goals.
This includes encouraging them to be actively engaged in promoting their health.
Responsibilities:
Assist patients in specific communities or with targeted populations to help manage social determinants of health (SDOH) and promote favorable health outcomes Work closely with MyCare primary care providers, members of the interdisciplinary care team, and community agencies to help improve patient care and access to care Build trusting relationships with patients, connecting them to social and primary care, while also helping eliminate barriers Help patients and their families access, navigate, and coordinate community services and social care resources to promote self-management, independence, and adoption of healthy behaviors Support providers and the Care Managers through an integrated care approach, bridging care management and community outreach
Qualifications:
High School Diploma required Minimum of 1 year of experience working as a Community Health Worker (CHW) OR 2 years of experience working with community resources, social programs, or in a medical health office Community Health Worker (CHW) certification preferred (or must be willing to complete upon hire) Bilingual Spanish & English is preferred but not required EHR experience required Effective oral, written communication, and telephone skills Good organizational skills to handle multiple priorities while remaining professional and calm Ability to work with many diverse people, including elderly disabled, and underserved populations Ability to work independently, be self-directed and flexible Ability to perform functions with minimal supervision Job Type:
